Newbie here, I started Glenmark just over 2 weeks ago 1mg. Dosing 1 x week, bumped with grapefruit this last time. Not sure what I am supposed to feel / observe and when to indicate it is working.

Most people don’t feel any different on Rapamycin.

I take Pfizer Rapamune and Zydus Siromus. Zydus seems to be stronger in that I get more side-effects.

Very interesting and thanks for sharing this! It has been a little hypothesis that I have that different brands can have more or less side-effects. What type of side-effects do you get more of when taking Zydus?

All of the side effects I have had are from Zydus. I started Rapammune and moved on to Zydus. Then I developed side effects. I spent 5 weeks on the Pfizer brand. 4 weeks at 1 mg + GFJ and the last week 2 mg + GFJ. Zydus was 2 mg + GFJ weekly after that.

My last prescription of sirolimus with CVS in Sept ‘25 using GoodRx was a generic by Greenstone LLC. Told the pharmacist I wanted a US manufacturer. The pill is fat triangle with the words Rapamune and 1mg. Walmart filled the prescription in early ‘25 with Dr Reddy’s. I could tell that my teeth were not as smooth and clean feeling with Dr Reddy’s with the same dosage.I didn’t lab test for purity but went on and finished the prescription. From my experience, if you don’t request a US manufacturer up front CVS also gives Dr Reddy’s for sirolimus and other prescriptions I take as well.

The only US manufacturer is Pfizer rapamune and it’s 5 to 10 times more expensive as it’s the brand name drug.

All the generic versions of rapamycin are manufactured by Indian pharma companies in India.

Greenstone LLC is a US manufacturer and subsidiary of Pfizer. It makes generic sirolimus. I didn’t know they used a plant in India to manufacture the pills. I have read of FDA quality control issues with the DrReddy plants in India. Greenstone’s India plant I assume is manufacturing a sirolimus product with the same proprietary formula identical to non generic Rapamune produced in the States. The Greenstone pill face is “Rapamune 1mg”. I haven’t read of any FDA concerns relative to Greenstone’s outsourcing to India.
So Glenmark, as well, a US pharmaceutical company making generic sirolimus is also using plants based in India ro manufacture the product. For 100 pills with Greenstone brand the cost was 180.00 with GoodRX at CVS.

Wow - you are absolutely correct, my bad. I’ve never seen these. See photo below, I think you’re right. If you can get that version of the pill its probably one of the best versions of sirolimus out there - I’ve never seen it before. I’m not sure where its manufactured - but perhaps if you dig around you can find out. I would tend to agree that a Pfizer subsidiary probably has fewer quality issues than the typical generic Indian Pharma manufacturer. I’m assuming Glenmark manufactures in India - but I’m not sure.

The Dr Reddy’s pill has RD 53 on the triangular pill.face. Is Rapamune a trade protected Pfizer name. The Greenstone pill says Rapamune.

If you are using a prescription and want generic it’s best to call the pharmacy ahead and explain you want this Greenstone brand. Don’t let the pharmacy choose the manufacturer for you or you will get the more available DrReddy’s.

You are right - the generic Rapamune (Pfizer) is no longer made in a US facility. Statement on Pfizer’s decision on the brand drug, Rapamune, in the U.S. - The LAM Foundation
